Provide a letter of introduction and resume for the fieldwork site including strengths and challenge areas (sample on website)

Confirm time and location for arriving at placement on first day, at least two weeks prior to placement

CPR/IMMUNIZATIONS/MEDICAL EXAM/CRIMINAL RECORD CHECKS

All fees and other costs incurred for Cardio-pulmonary Resuscitation (CPR) certification, re-certification (which must be completed annually), medical examinations, immunizations and criminal record checks are the sole responsibility of the student.

 Placements will not be assigned unless the Clinical Education Assistant has received the following:
• Photocopy of Basic Rescuer Level CPR certificate (annual recertification)
• Medical Exam including a copy of immunizations

Students must have a medical examination prior to the first placement. A Medical Examination Form is provided and must be completed, signed and returned to the Fieldwork Assistant by the designated date. Any change in a student’s medical status must be brought to the attention of the Academic Coordinator of Clinical Education prior to any subsequent fieldwork placements. An assessment will be made for the need of medical verification prior to the next placement.

It will be the student’s responsibility to ensure up-to-date immunizations on a yearly basis and to carry current immunization records to each placement. Times available for TB tests will be posted at the beginning of each year. Failure to comply may result in cancellation of a placement.

Placements may require a criminal record check. The sites determine the criteria for acceptance/denial of placements.

STUDENT RESPONSIBILITIES DURING PLACEMENT
 
 
bulletIt is the student’s responsibility to ensure that she/he graduates with fieldwork experience in a diversity of settings and with a range of age groups and diagnoses. At a minimum, a student must complete 150 hours of fieldwork in each of the following areas: mental health, physical health, and a community and/or rural setting.

SUMMARY CHECKLIST

Report to the fieldwork site at the time and location appointed.

Maintain professional appearance: the dress code of the facility must be followed.

Reviewing and adhering to the national (CAOT) and provincial professional codes ( AAROT, SSOT ) of ethics, and to site policies.

Maintain confidentiality in all matters related to clients and the facility. This is important both in oral and written communications.

Students’ working hours (including lunch, coffee breaks, daily work schedules, etc.) are determined according to the policies of the site. Fieldwork hours are calculated on a 37.5 hour week.
Fill out a copy of the CBFE to evaluate his/her own performance and share these results with the preceptor at both mid-term and final evaluation

FIELDWORK PLACEMENT ATTENDANCE

100% attendance by students in fieldwork placements is mandatory; There is a zero tolerance policy towards any hours missed. Students must make up all time missed regardless of the reason.

The Academic Coordinator of Clinical Education and the preceptor must be notified and a strategy determined regarding making up hours. Special arrangements will be made to complete the hours missed.

All hours must be completed or a grade of "incomplete" (IN) will be earned. We encourage students to document extra hours accrued at each site for inclusion in their professional portfolio.

PROJECTS

A facility may determine that an assignment would augment the learning experience, and assist the student to achieve the fieldwork objectives of the placement. It is recommended that the assignment demand no more than 5% of the placement time,
i.e. 5 weeks =10 hours.
